Well, Shinji didn't literally kill anyone (except Kaworu...) he just didn't care if it happened or not. He probably didn't even know that his desires were going to be made into reality by Lilith (or if he'd just descended into complete psychosis), and plenty of people in such despiring situations have thought to themselves "I wish everyone were dead!"
